Elysian’s Virtual Intake Tool

You will be scheduling a 60-minute virtual (telehealth) intake appointment through our online platform, SimplePractice. You’ll receive an email from SimplePractice (yourprovider@simplepractice.com) with your intake and consent documents. These must be completed at least 24 hours before your appointment.

The normal self-pay cost for the first individual therapy session is $170. Each recurring session thereafter is $150. For couple’s and family therapy, all sessions are $175. The next section will explain how to use your insurance.

We welcome in-network patients with Aetna, United Healthcare, Blue Cross Blue Shield, CareFirst, Oxford, and Oscar Health. Once you book your appointment, our team will add you to our billing platform, Headway. You’ll receive a welcome email from Team Headway and access their system to sign their consent forms, complete your demographic information, and put a method of payment on file.

You will receive an email and text from SimplePractice no later than 10 minutes before your appointment. Simply click the link on a laptop or desktop computer to enter the session. If you wish to join from a phone or tablet, download the SimplePractice app for Apple or Android.

While we do understand circumstances can arise that prevent attending your appointment, we do request as much advance notice as possible in the event you are unable to participate. Cancellations made less than 24 hours before the appointment’s start time will automatically be assessed a $100 cancellation fee. Clients that are a “no-show,” or fail to provide notice that they will not attend, will be charged the full standard practice cost of the appointment as noted in the “Payment” section above.
